The TBI Waiver promotes independence through person-centered planning, where each service plan is customized to meet individual needs. It fosters community involvement by encouraging participation in local programs and activities. Most importantly, it emphasizes self-determination—empowering individuals to make informed choices about their care and daily life.
Haven Point Home Care is committed to helping individuals enrolled in the TBI Waiver live safely and independently in their communities. We develop individualized care plans focused on personal goals, connect clients with trained caregivers for hands-on support, and collaborate closely with healthcare professionals to maintain continuity of care. Our team also provides clear guidance and advocacy to help families navigate the waiver process with confidence and peace of mind.
While the TBI Waiver offers valuable support, there are important factors to consider. Service availability may vary by region, leading to unequal access. Program limitations, such as caps on hours or funding, can affect the level of care provided. Additionally, navigating the application and approval process can be complex, often requiring guidance and persistence.
The TBI Waiver Program provides essential resources for individuals recovering from traumatic brain injuries, allowing them to remain safe and engaged within their communities. Through individualized services, skilled support, and opportunities for community participation, the program helps promote independence and stability. Participants must be Medicaid-eligible adults between the ages of 18 and 64 who require a nursing facility level of care due to a brain injury. Haven Point Home Care is honored to support individuals throughout this process, offering knowledgeable coordination, compassionate caregiving, and reliable assistance at every stage.
