To qualify for New York’s Nursing Home Transition and Diversion (NHTD) Waiver, applicants must meet the following criteria.
Applicants must be at least 18 years old. Individuals 65+ may qualify without additional disability documentation.
Applicants ages 18–64 must provide proof of physical disability through SSI/SSDI awards, medical evaluations, or official agency records.
Must be enrolled in Medicaid and meet nursing-facility level of care requirements through a state-approved assessment.
Applicants must live—or plan to live—in a private home, apartment, or licensed adult-care setting (not an institution).
Must meet New York State Medicaid income and asset limits.
