There are two main models for receiving Personal Care Assistance: Traditional PCA and PCA Choice.
In the Traditional PCA model, a home care agency hires, trains, schedules, and supervises the PCA staff—handling payroll, billing, and other administrative tasks.
In the PCA Choice model, the consumer has more control. You recruit, hire, train, and schedule your own caregivers, while a fiscal intermediary manages payroll and billing.
Traditional PCA works best for those who prefer agency-managed support, while PCA Choice is ideal if you want more flexibility and say in who provides your care.

What’s the difference between Home Health Aide (HHA) and Personal Care Aide (PCA) services?
An HHA is a certified aide who provides care and support for elderly and/or disabled patients who need help completing daily functions. HHAs may check vital signs (such as pulse and blood pressure) and help with bathing, dressing, grooming, mobility, and other routine activities.

New York offers Personal Care Services under Medicaid State Plan, including Level I housekeeping and Level II personal care/ADLs, along with self-directed models such as CDPAP.
Choose between agency-provided PCA through licensed home care agencies or self-directed care (like CDPAP), where you select and train your own assistant and a fiscal intermediary handles payroll and billing.
